It is very important that the Human Resources Office is informed of any changes employees have to their personal information. These changes include address and phone number changes, name changes, changes in marital status, and emergency contact information changes. If an employee has a change such as these, they should complete a Personal Data Sheet change form and provide it to the Human Resources Office.
