Florida Retirement System (FRS) Beneficiary Designation |

This message is intended for regular full and part-time employees in the Florida Retirement System.
Beneficiaries of FRS members are eligible for a refund of employee contributions in the event of the employee’s death. Employees should ensure they have designated a beneficiary by reviewing their User Profile at www.myfrs.com (instructions to create a User Profile are below), reviewing their FRS account statement received in the mail or calling the FRS Financial Guidance Line at 1-866-446-9377. If you have not designated a beneficiary, please follow the instructions below.
To designate a beneficiary, employees have two options:
1. Login to online services at www.myfrs.com . Employees must create a User Profile in order to add a beneficiary. To gain access to create a profile, members must use their social security number and date of birth.
2. Employees may print the beneficiary designation forms by going to www.myfrs.com then click on ‘Investment Plan Beneficiary Form’ or Pension Plan Beneficiary Form’ (depending on which plan you are in) located under Frequently Used Forms on the right side of the homepage. Instructions for submitting the completed form are located at the bottom of designation form.
