2014 HEALTH PLANS

Retirees of Edison State College may choose from a variety of BlueCross and Blue Shield of Florida health plans. All retirees and their dependents covered through BCBSFL have access to the Member Services program which allows members to review claims, order replacement cards and view the Provider Directory.

GRANDFATHERED HEALTH PLANS

Grandfathered Health Plans are health plans that the College offered prior to 2014 and will continue to offer throughout 2014. The health plans the College offers meet the grandfathered status required for Health Care Reform Compliancy.

Full-time employees may elect dental insurance for themselves and their dependents through Delta Dental. Two dental plans are offered, Delta Dental PPO Plan and Delta Care USA (HMO) Plan. Dental insurance is a voluntary benefit paid for by the employee. Coverage is effective the first of the month following date of hire and premiums are paid one month in advance.

***If you haven’t received or lost your dental card the provider can verify coverage through Delta Dental Customer Service (Dental PPO plan: 1-800-521-2651 Group #: 16020, Division: 12004, Dental HMO plan: 1-800-422-4234, Group #: 76722, Division: 0001).

Vision Plan  Plan Summary  |  Monthly Rates

Full-time employees may elect vision insurance for themselves and their dependents through VSP Vision plan. This is a PPO Vision plan, so in and out of network benefits are available. Informational booklets and enrollment forms are available in Human Resources or you can view additional information at www.vsp.com. Vision is a voluntary benefit paid for by the employee. Coverage is effective the first of the month following date of hire and premiums are paid one month in advance.

Life Insurance Life Insurance Certificate  |  Evidence of Insurability Form
The College provides a $15,000 basic term life insurance policy through Unum Group Life, including accidental death and dismemberment, for all full time regular employees at no cost to the employee. Coverage is effective the first of the month following date of hire. In addition, employees can purchase supplemental life insurance at 1, 2, or 3 times their base salary. The cost is $.277 per $1000.00 of coverage per month. Supplemental life insurance requires application and approval from the insurance company. Supplemental Life Insurance applications are available in the Human Resources office. Coverage effective date is determined by the insurance company and premiums are paid one month in advance.

It is important for Retirees to keep us informed about any changes. For address, phone number or emergency contact changes please complete a Personal Data Sheet and submit it to Human Resources.

If you would like to change your Beneficiary for Life Insurance, please contact Human Resources to receive a Beneficiary Change form.
